Introduction

This case represents the importance of diabetes control in infection control and cure along with medical therapy, nutrition, education, and monitoring .

Background: Severe Hyperglycemia and infection Challenges

20yrs old girl came with newly diagnosed type 2 diabetes with an HbA1c of 12.6% ,FBS- 265 ,PLBS – 338 and she was also diagnosed with pulmonary koch’s TB ( tuberculosis )recently and she was having a lot of symptoms like fever, infection and weight loss.she needed a structured, sustainable and scientific management plan.

Our Approach

We developed an integrated diabetes program which included: – Individualized insulin therapy for her, which was very important because in infections like tuberculosis , very good and strict control of diabetes is very necessary. So we initiated insulin twice a day for her along with oral medication ,good diet therapy, adequate protein intake and also good self-monitoring regime for controlling her diabetes .

Recovery and Outcome

The patient in span of just 1 month achieved a good diabetes control of FBS 265 mg/dl to 122mg/dl and PLBS 338mg/dl to 137mg/dl and her infective symptoms ,cold,cough,fever also subsided completely. She continued with self monitoring. After 3 months her Hba1c also reduced from 12.6% to 7.7%. All this helped the infection of tuberculosis to be controlled well .She is going to take the Tuberculosis treatment for a priof of 9 months.

Doctor’s Perspective

This case reinforces the importance of multidisciplinary diabetes care in any infection related case , like tuberculosis or urinary tract infection . we need insulin for good and strict aggressive diabetes control along with good nutrition.
